Can I use it for my shoulders?

Yes, it is better then heating pads, i purchsed it for my neck and shoulder. I prefer to use ThermaChill, a pain reliever just before i use this pillow massager. It works great.

I have upper back and shoulder pain mostly and work in an office and am wondering if this device works well in an office chair?

Yes, absolutely! You may need an extension cord for a longer reach, but it is light and very adaptable to the lower, upper back and shoulder. Great for the neck, lower cranial area and front torso. It is discreet enough for the an office chair. Soon, your co-workers will want one too!

I'm looking for a massager that does it all: calfs, upper legs, lower and upper back, shoulders, and neck. Does this massager work for all that?

I have a short neck so I have to keep moving it to get a good neck massage. But legs, upper and lower back, yes definitely a great massager - the best one I've purchased.
